1. Grooming Risks & Release of Liability
I understand that grooming may expose my pet to certain risks, including but not limited to: cuts, nicks, scratches, stress, anxiety, allergic reactions, and aggravation of pre-existing medical conditions. I release All 4 Paws LLC and its staff from any liability related to these risks, provided reasonable care and precautions are taken.
If an injury directly related to grooming requires veterinary care, All 4 Paws LLC will cover the cost, provided it is reported and treated within 72 hours.
2. Health & Vaccinations
I confirm that my pet is up to date on the rabies vaccine, as required by state law, and can provide a current certificate.
I will not present a pet for grooming that has fleas, ticks, contagious conditions, pre-existing health conditions or aggressive behavior without disclosing this information in advance.
If my pet shows signs of a contagious condition (such as ringworm, kennel cough, giardia, worms, or blood in stool) while in the care of All 4 Paws, I agree to pick up my pet immediately.
3. Emergency Medical Authorization
In the event of an emergency, if I cannot be reached immediately, I authorize All 4 Paws to transport my pet to the nearest emergency veterinarian and authorize the veterinarian to provide treatment as necessary.
Emergencies directly related to grooming services (such as lacerations or grooming injuries) will be covered by All 4 Paws.
Emergencies related to other medical or behavioral conditions (such as seizures, strokes, or breathing issues) will be at my expense, and I will not hold All 4 Paws liable.
4. Matted Coats & Special Handling
I understand that removing mats may increase the risk of skin irritation, hematomas, cuts, or changes in behavior.
I acknowledge that All 4 Paws will use professional judgment regarding de-matting or shaving, prioritizing my pet’s safety and comfort. I also understand that severely matted coats require additional time and care, and extra fees may apply.
5. Aggressive or Difficult Pets
If my pet becomes aggressive, dangerous, or too stressed to safely complete the groom, I understand that the service may be stopped.
The groomer may suggest:
-Completing the groom in multiple sessions
-Scheduling a private appointment
-Consulting with a veterinarian about medication
All 4 Paws prioritizes the pet’s comfort and stress level. Additional handling fees may apply.
6. Late Pick-Up/ Drop off & Cancellations
I agree to contact All 4 Paws if I am going to be more than 5 minutes late to my pets appointment time. I acknowledge that I may have to reschedule if I am going to be more than 15 minutes late, or my pet may need to stay longer, depending on how busy the schedule for the day is.
I agree to pick up my pet at the designated time. Late fees may apply, up to $1 per minute after closing.
I understand that missed or canceled appointments without 24-hour notice may be subject to a cancellation fee or require a deposit for future bookings.
7. Customer complaints
I acknowledge that if I am not satisfied with any aspect of my pets groom or experience, I will notify All 4 Paws within 48 hours. We are committed to addressing and resolving your concerns to the best of our ability.
8. Agreement
I have read and understood this release form. I agree to hold harmless All 4 Paws LLC, its owners, employees, and agents from any liability, claims, or damages arising from grooming services.
